The Visitation Aide will work alongside a multidisciplinary team providing supportive visitation services to DCPP involved families who are working toward a successful reunification.

The Children’s Outpatient Services Clinician will work with young adults age 5 to 21 who are at risk of psychiatric hospitalization. The Clinician will document and deliver evidenced-based individualized treatment to youth and their families in a community-based outpatient setting.
Interview and determine eligibility for rental/housing assistance, provide case management services to at-risk or homeless households in Middlesex County, includes service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers/landlords, housing retention, completing housing inspections throughout NJ, and utilization of HMIS case management software.
Visitation will occur in the least restrictive setting determined for each family’s unique needs, in order to facilitate a successful family reunification. Therapeutic Visitation Specialists will be trained in an evidence-based model, and will complete initial intake assessment tools, conceptualize personalized visitation plans, facilitate debriefings before and after visits, facilitate visitation planning meetings, and complete visitation documentation and reports.
