Provide case management services to people in Permanent Supportive Housing including completion of intakes, developing service plans, linkages to other services including, conflict de-escalation, managing relationships with a broad range of community providers, promoting housing retention through teaching life skills, setting up programming for tenants
Utilization of HMIS case management software
Minimum Requirements:
BA/BS
Bi-lingual encouraged to apply. Ability to work independently
May include additional on-call hours
This position has evening hours 2 nights a week and flexibility in creating own work schedule
Driver’s license with acceptable driving history
