Responsibilities

• Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Middlesex County
• Provides in-home counseling to CP&P referred families.
• Case management and case documentation
• Case conference with CP&P and other providers
• Knowledge and experience in CBT and Motivational Interviewing preferred

Requirements

• Rotating 24/7 coverage required
• Master’s Degree in Social Work or related field with licensure as applicable to degree
• Flexible schedule including availability on nights
• Driver’s license needed with an acceptable driving history
