Hunterdon/Warren Behavioral Health & Youth Services, Phillipsburg, NJ

• Work as part of a multi-disciplinary team grounded in the housing first approach and coupled with an integrated service model to assist families in obtaining and retaining housing in Warren County
• Responsible for establishing a trusting relationship with families and working with them to identify their service goals
• Utilize holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services
• Case management and case documentation
• Case conference with CP&P and other providers
• 24/7 rotating coverage

Minimum Requirements:

• Bachelor’s degree in related field required and 2 years related experience. 
• Requires a flexible schedule including availability on evenings and weekends. 
• Valid driver’s license with an acceptable driving history.
