Responsibilities

• Works as part of a multi-disciplinary PACT team
• Provides community-based treatment and skills training to persons with serious mental illness
• Responsible for identifying, locating, and securing housing for individuals with mental illness
• Works closely with state psychiatric hospitals to facilitate discharges of individuals into the community

Requirements

• BA/BS/BSW
• Minimum of 2 years working with the mentally ill population OR a Master’s degree and one year post-bachelor’s experience with the mentally ill population
• Experience in housing options for difficult to place individuals preferred
• Valid driver’s license with an acceptable driving history
