Screening, assessment, service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers, life skills teaching, transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies.
Utilization of HMIS case management software.
Ability to work independently.
On-call coverage and evening hours required.
Bi-lingual encouraged to apply.
Requirements
Bachelor’s degree from an accredited college, preferably in Social Work.
Experience with conflict resolution and mediation.
2-years relevant work experience and demonstrated ability to provide effective services to clients required.
Needs to be culturally competent across all populations. Bilingual a plus.
Driver’s license with acceptable driving history.
