Case Manger - Part-time Edison, NJ

Responsibilities

• Provide case management services to individuals residing in Permanent Supportive Housing including completion of intakes, developing service plans, linkages to other services including, conflict de-escalation, managing relationships with a broad range of community providers, promoting housing retention through teaching life skills, setting up programming for tenants
• Utilization of HMIS case management software
• Ability to work independently

Requirements

• May include additional on-call hours
• This position has evening hours 2 nights a week and flexibility in creating own work schedule
• Driver’s license with acceptable driving history
• BA/BS - required
• Bi-lingual encouraged to apply
