Responsibilities

• Provide case management services to individuals with mental health illness, who are at imminent risk/currently homeless
• The job includes screening, assessment, community outreach across the county, service planning, linkage, counseling, life skills teaching and utilization of HMIS case management software

Requirements

• This job requires on-call availability, after-hours coverage, evening hours, and a tenacious and outgoing personality
• Preferably bi-lingual
