• Provide case management services, multi-family psychoeducation and support group facilitation, advocacy linkages and referrals
• Ensure proper documentation in timely and consistent manner

Minimum Requirements:

• BA/BS in social work, psychology or related degree with a minimum of 2 years’ experience in case management and 2 years’ experience working in the mental health field. 
• Flexible schedule as some duties require evenings. 
• Valid driver’s license with an acceptable driving history.
