• Provide case management services, including outreaches, linkages, advocacy, referrals and general support
• Multi-family psychoeducation and support group facilitation, including researching ongoing psychoeducation education topics and presentations
• Provide ongoing and communication with open caseload
• Ensure proper documentation in timely and consistent manner

Minimum Requirements:

• BA/BS in psychology or related degree with a minimum of 2 years’ experience as a case manager and 2 years’ experience working in the mental health field. 
• Flexible schedule as some duties require evening hours. 
• Valid driver’s license with an acceptable driving history.
