Responsibilities

• Provide case management services to individuals at Ozanam Inn Shelter
• Includes screening, assessment, service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers, life skills teaching, transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies and utilization of HMIS case management software

Requirements

• On-call coverage and evening hours
• Ability to work independently
• This position requires a driver’s license with acceptable driving history and BA/BS (requirement)
• Bi-lingual encouraged to apply
