Family Specialist 099 – Bridgewater, NJ

Responsibilities:

• Works as part of a multi-disciplinary team grounded in the housing first approach and coupled with an integrated service model to assist families in obtaining and retaining housing in Somerset, Hunterdon and Warren Counties.
• Responsible for establishing a trusting relationship with families and working with them to identify their service goals.
• Holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services.
• Rotating 24/7 coverage required.
• Case management/case documentation.
• Case conference with CP&P and other providers.

Requirements:

• Bachelor’s Degree in related field required with 2 Year's related experience.
• Requires a flexible schedule including availability on evenings.
• Driver’s license needed with an acceptable driving history.
• Remote work possible after 3-month probationary period.
