Responsibilities

• Establishing trusting relationships with families and working with them to identify their service goals
• Using a holistic planning approach to assist families with connecting and coordinating needed health and social services
• Rotating 24/7 coverage is required. Case management and case documentation are essential, along with case conferences with CP&P and other providers

Requirements

• Bachelor’s Degree in Social Work with a minimum of two (2) years related experience, preferred
• Requires a flexible schedule, including availability on nights and weekends
• A valid driver’s license with an acceptable driving history is required
