Work as part of a multi-disciplinary team grounded in the housing first approach and coupled with an integrated service model to assist families in obtaining and retaining housing in Hunterdon County
Responsible for establishing a trusting relationship with families and working with them to identify their service goals
Utilize holistic planning approach to assist families with connecting to, navigating and coordinating needed health and social services
Case management and case documentation
Case conference with CP&P and other providers
24/7 rotating coverage
Minimum Requirements:
Bachelor’s degree in related field required and 2 years related experience.
Requires a flexible schedule including availability on evenings.
Valid driver’s license with an acceptable driving history.
