Middlesex Behavioral Health & Youth Services, Perth Amboy, NJ

• Responsible for providing community based case management and clinical services to a caseload of youth age 17-21, who are transitioning out of resource home care
• Provide services to address the needs of young people with individualized skill development and support to transition successfully into adulthood
• Specific needs include obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system and remaining free from legal involvement
• Participates in evidenced based model utilizing cognitive behavioral therapy and motivational interviewing and is expected to adhere to this model throughout the intervention
• Works in the field and participate in a 24/7 on-call rotation

Minimum Requirements:

• Bachelor’s Degree in social services field plus one year of experience required. 
• Master’s degree and bilingual (English/Spanish preferred.  
• Strong organizational, knowledge of the community and the ability to advocate for client needs. 
• Ability to work independently as well as part of a team.  
• Valid driver’s license with an acceptable driving history.
