Middlesex Behavioral Health & Youth Services, Perth Amboy, NJ
Responsible for providing community based case management and clinical services to a caseload of youth age 17-21, who are transitioning out of resource home care
Provide services to address the needs of young people transitioning out of care and need individualized skill development and support to transition successfully into adulthood
Specific needs include obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system and remaining free from legal involvement
Participates in evidenced based model utilizing cognitive behavioral therapy and motivational interviewing and is expected to adhere to this model throughout the intervention
Works in the field with access to an agency vehicle and laptop and participate in a 24/7 on-call rotation
Minimum Requirements:
Bachelor’s Degree in social services field plus one year of experience required.
Master’s degree and bilingual (English/Spanish) preferred.
Strong organizational, knowledge of the community and the ability to advocate for client needs.
Requires a flexible schedule.
Ability to work independently as well as part of a team.
Valid driver’s license with an acceptable driving history.
