Responsible for managing a team of 4 Specialists who provide community based clinical and case management services to youth age 17-22, who are transitioning out of resource home care and need individualized skill development and support to transition successfully into adulthood
Specific needs include obtaining and maintaining safe and suitable housing, continuing their education, leaning independent living skills, obtaining and maintaining employment, building a support system and remaining free from legal involvement
Participates in evidenced based model utilizing cognitive behavioral therapy and motivational interviewing
Ensure fidelity to the model and provide weekly supervision and consultation
Works in the field and participate in a 24/7 on-call rotation
Minimum Requirements:
Master’s Degree in Social Work, Psychology or Human Service field required.
Bilingual (English / Spanish) and Clinical license (LSW, LPC or LCSW) preferred.
Must have supervisory experience and demonstrate effectiveness in leading a team.
Strong organizational and interpersonal skills.
Ability to interact with stakeholders and community providers.
Valid driver’s license with an acceptable driving history.
