The Volunteer and Community Partnership Coordinator works closely with the Middlesex County community and families to identify community/family needs, identify resources, and build mutually beneficial relationships with all parties.

We are seeking a LAC or LSW for a part time employment opportunity to work with young adults ages 5 to 21 years of age. The Clinician will document and deliver evidenced-based individualized treatment to youth and their families in a community-based outpatient setting.

Interview and determine eligibility for rental/housing assistance, provide case management services to at-risk or homeless households in Middlesex County, includes service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers/landlords, housing retention, completing housing inspections throughout NJ, and utilization of HMIS case management software.

Provides community based treatment and skills training to persons with serious mental illness
