Provide case management services to individuals with mental health illness, who are at imminent risk/currently homeless. The job includes screening, assessment, community outreach across the county, service planning, linkage, counseling, life skills teaching and utilization of HMIS case management software.

Credentialed Substance Use Counselor to be primary clinician for Level 1 outpatient substance use program in Phillipsburg. May conduct full substance use assessments, make recommendations for level of care, facilitate individual and/or group counseling sessions, complete treatment planning, discharge planning, as well as case management with referral sources for mandated clients.
