Transportation of children and parents to and from their scheduled visitation utilizing an Agency vehicle in Middlesex and Union Counties in both office-based and community-based settings. Provides occasional supervision of visits.

Full time Career Coach working as part of a multi-disciplinary team to provide services to young adults (ages 18-24) in a transitional living program. Provide targeted employment and education case management in Somerset and Middlesex counties; supporting youth in developing goals, and obtaining and maintaining a living wage.

Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Hunterdon, Mercer, Somerset and Warren Counties. Provides in-home counseling to CP&P referred families

Visitation will occur in the least restrictive setting determined for each family’s unique needs, in order to facilitate a successful family reunification. Therapeutic Visitation Specialists will be trained in an evidence-based model, and will complete initial intake assessment tools, conceptualize personalized visitation plans, facilitate debriefings before and after visits, facilitate visitation planning meetings, and complete visitation documentation and reports.

Work with Somerset County families and/or care givers with an adult relative living with mental illness. Conduct assessments, provide support and resources, coordinate and conduct support groups and psycho-educational groups.

Provide case management services to individuals residing in the Ozanam Inn Shelter and Permanent Supported Housing. The job includes screening, assessment, service planning, linkage, crisis intervention, and de-escalation, managing relationships with a broad range of community providers, and life skills teaching
